Community Support Workers (CSWs) to join our
ACT Plus team. In this role, you'll provide hands-on support to adults in the community, helping ensure their safety, supporting medication needs, and encouraging greater independence and community involvement.

ACT Plus program of Resources for Human Development (RHD) provides community-based support to adults who benefit from individualized assistance and services.
As a Community Support Worker, you'll build relationships with the individuals you support while helping promote safety, independence, and participation in the community.

What You'll Do As a
Community Support Worker , you will provide direct support to adults, including assistance with medication and ensuring the safety and well-being of the individuals receiving services.
Your Responsibilities May Include: Individual Support & Independence - Develop positive, supportive relationships with the individuals you serve.
- Encourage individuals to become involved in their communities.
- Promote independence while providing appropriate individualized supports.
- Assist in maintaining the safety and well-being of individuals receiving services.
Medication Support - Assist with the planning and preparation of medication routes as required.
- Assist individuals with medication administration as necessary.
- Complete Medication Administration Records as required.
- Follow established medication policies, procedures, and applicable requirements.
Safety & Crisis Support - Recognize and report crisis situations or potential concerns to appropriate supervisors and treatment providers.
- Provide crisis intervention as needed.
- Report suspected incidents of neglect, abuse, or mistreatment in accordance with agency policy.
- Promote a safe and supportive environment.
Documentation & Team Support - Maintain accurate required records and documentation.
- Maintain confidentiality in accordance with agency policies.
- Complete vehicle logs and follow agency requirements regarding vehicle use.
- Assist with office and program-related tasks as assigned.
- Collaborate with team members to ensure individuals receive consistent, quality support.
- Perform additional duties as requested or assigned.

Qualifications Required - High School Diploma
- Valid Driver's License in good standing
- At least 3 years of continuous driving experience
Training & Certifications Required training and certifications include:
- CPR/First Aid
- MANDT Training
- Assistance with Self-Administration of Medication
- Required agency orientation and training
Schedule & Location - Part-Time
- $15.00/hour
- Approximately 4-hour shifts
- Morning Shift: 8:00 AM to 12:00 PM
- Evening Shift: 4:00 PM to 8:00 PM
- Opportunities may be available Sunday through Saturday
This position is ideal for candidates looking for consistent part-time hours in either morning or evening shifts.
Physical Requirements - Ability to walk, sit, and stand throughout the shift.
- Ability to assist individuals with light physical activity as needed.
- Ability to perform essential job functions with or without reasonable accommodation.
